The Public Health Guidelines require that this declaration form be completed by all involved in the National Fleadhfest Sligo, including competitors, nominated person accompanying competitors, participants, performers, volunteers, contractors, contractors employees, workers etc. it is to be completed within a period of 3 DAYS of attending at the Fleadhfest venue. Failure to complete this form will mean that you cannot be involved in National Fleadhfest Sligo in any capacity, role or activity.
